Keith Richards used drugs for a long time. The guy is not stupid but he did become a junkie. He saw people around him suffer a lot because of bad drugs, unscrupulous dealers.
What he did was to make sure he only got the best drugs you could possibly get, drugs made by the government. Yes, the government who also is at war with drugs, also makes some narcotics. He found people who had been part of a brief experiment or something where they would get drugs made by the government for life. It is explained in his book.

Now, I don’t use illegal drugs and narcotics are so illegal where I live that I never wanted to try them. That is me. I am not Keith Richards. But basically the quality of the government drugs is so good that you don’t get hangovers. The drugs do their fun part and don’t poison you. They are not cut with whatever, not made with petrol and nasty ingredients. They are made by people in a lab with top high end equipment. The last thing the government wants is to kill their patients.
So, he could use drugs which basically only a handful of other people could use, made to perfection.
But he also has one other thing that you and I will not have, Mick Jagger.
Mick Jagger quit drugs in the 1980s, completely. He trains like an ironman. He runs long distances, eats right and really takes care of himself. He cleaned up Keith when he was ready and willing to be cleaned up.

So, his drug use did not hurt him all that much and his alcohol abuse did probably hurt him a bit, also his smoking surely did not help.
He lives a relatively clean life in comparison today. Takes good care of himself.
He also has good genes which have made it possible for him to get a lot cleaner and not suffer as much as others in his position would have suffered.
